The Diabetes Self-Management Education (DSME) program is a service offered by South Fulton Medical Center. The program is recognized by the American Diabetes Association for meeting National Standards for DSME programs. We offer management education to adults with Type 1 and Type 2 diabetes, as well as gestational diabetes, pre-diabetes and metabolic syndrome. The goal of the program is to help individuals with diabetes learn about diabetes self-care so that they will be able to better manage their diabetes.
Diabetes Center at South Fulton Medical Center
South Fulton Medical Center’s Diabetes Education Program recently was awarded the American Diabetes Association (ADA) Education Recognition certificate for a quality diabetes self-management education program.
South Fulton Medical Center offers diabetes educational services on
both an inpatient and outpatient basis. Our goal is to provide patients
with comprehensive diabetes information that ensures positive
outcomes throughout the healthcare continuum. We want to ensure
that you and your family can receive quality care close to home.
Diabestes Education Classes
Classes are held from 9 a.m. to 4 p.m. on weekdays. Individual
education sessions can be ordered by your physician if you have
special needs. Participants should report to main admissions,
located on the first floor, at 8:30 a.m. the day of the class.
A physician’s referral is required to sign up for a class.
Instruction is provided by a Board Certified Diabetes Educator, who
is a Registered Nurse, along with a Registered Dietitian.

Class Registration
Participants enter the program through a referral from their physician. Appointments are made by calling 404.466.1330. The referring physician is asked to fax the signed referral form to Admissions at 404.466.1375.
